Parent Acknowledgement and Responsibility for Equipment/Uniforms Issued

 

 

I am aware that my son will be issued football equipment and uniforms in August which includes the following:

 

 

Helmet, Chin Strap, Mouthpiece

Shoulder Pads

One to Twp Practice Jerseys

One Pair Practice Pants

One Practice Belt

Two Thigh Pads

Two Hip Pads

Two Knee Pads

One Butt Pad

 

Varsity Uniform

Two Away Jerseys

One Home Jersey

Three Pair Game Pants

One Game Belt

Three Pair of Socks

 

JV/Freshmen Uniforms

One Away Jersey

One Home Jersey

One Pair Pants

One Game Belt (JV only)

One Pair Socks

 

I am aware that my son is responsible for all the above mentioned equipment and uniforms. It must ALL be returned and checked in when equipment is returned at the end of the season on the scheduled day. If you cannot return it on this day due to illness, it must be returned the following day or as soon as possible, in order to be cleared. If it is not, you will be billed for any of the missing equipment or uniforms. If your son quits prior to the season starting or during the season, he must return all equipment and uniforms which have been issued to him (which must be washed) to his head coach, we no longer have an equipment manager. NOTHING CAN BE LEFT IN HIS LOCKER.

 

If items are checked out and not returned, I AGREE to pay the replacement value of items missing, lost or stolen.
